REGISTRATION INFORMATION

Please note the following district registration requirements relating to evidence of age, residence and custodianship*. 

These are necessary before the student can start classes.

Proof of Age (only one is required) 

• Birth Certificate
• Adoption Certificate

Proof of Residency

• Mortgage statement
• Rent receipt
• Lease
• Closing statement with stamp
• Deed (satisfactory in cases in which there is a certainty that the parent/guardian and student actually reside in the district)

• Utility bill (telephones, etc. as soon as available)

• A letter showing service hook up with the utility company (cable, phone, gas/electric)
• Department of Motor Vehicle documentation i.e. Driver's License or Registration
• IRS Documentation

Approval by the School Superintendent must be achieved prior to registration of a student if parent/guardian cannot provide proof of residency.

Proof of Immunization

*Proof of Custody

Court papers which document custody/guardianship are required in all cases in which resident parent is divorced or separated.  It is only necessary to have a copy of title page, signature page, and that portion which documents custody or guardianship.

 Approval by the School Superintendent must be achieved prior to registration of any student residing in our school district with a non-custodial parent or other adult.  

If possible, please bring a copy of recent Standardized Test scores and the student's last report card.
