The New York State Legislature has amended the Civil Service Law to expand leave time with pay, which was previously limited to leave time for breast and prostate screening, examination and testing, to include screening, examination and testing for all cancers. Leave for this purpose may not be charged to sick leave or any other paid leave accrual of the employee, unless the period of absence on the workday of the screening exceeds four (4) hours, including travel time. Time taken in excess of four (4) hours would be unpaid time or charged to other paid leave accruals, if the criteria for use of such other paid leave is satisfied.
The above form is required to be filled out by your physician then submitted to Yvette Maag in the HR & Benefits office.
