IMMUNIZATION REQUIREMENTS FOR SCHOOL ATTENDANCE

Public Health Law Section 2164 and New York Codes, Rules and Regulations (NYCRR) Title 10, Subpart 66-1 require every student entering or attending public, private or parochial school in NYS to be immune to diphtheria, tetanus, pertussis (DTP), measles, mumps, rubella (MMR), poliomyelitis (IPV), hepatitis B, and varicella (chickenpox) in accordance with Advisory Committee on Immunization Practices (ACIP) recommendations.

If you receive notice from the school nurse that your child requires immunizations for school attendance, please send in proof that your child has received the required immunizations or make an appointment with their doctor. 

The notice you receive will have an exclusion date. Please provide proof of immunization before that date. If proof is not received by the deadline, your child will be excluded from attending school until they receive the mandated immunizations.

HEALTH EXAM REQUIREMENTS

New York State requires physical exams for students in grades K, 1, 3, 5, 7, 9, and 11, as well as for new students entering the school district for the first time. Physical exams must be completed on the required form. Forms can be provided by the nurse in your child’s building or printed from the provided link below.

If not done by your child’s physician, the following screenings will be completed by a school nurse.

•  Vision- Grades K, 1, 3, 5, 7, 11 and new entrants
•  Hearing- Grades K,1, 3, 5, 7, 11, and new entrants
•  Scoliosis- Boys grade 9,  Girls grades 5, 7

Working papers are available through the Nurse's Office during the academic year. Students can obtain applications from the Nurse's Office at both the Intermediate School and High School, or by visiting the following link:  Working Papers.

During the summer months, working papers can be obtained from the Main Office at both the Intermediate School and High School. For further information or assistance, please contact the Nurse's Office during the school year or the Main Office during the summer.
